Life insurance support

Are you the holder or beneficiary of a life insurance policy and you want to notify an insured event?

Contact the insurance intermediary or contact us directly, by email, at the address daune.viata@garanta.ro or at the phone number +4021.303.70.20 from Monday to Friday between 08:30 and 16:30 hours.

The occurrence of the insured event must be notified within 30 calendar days since its occurrence, by filling in and submitting the claim notification form specific to each type of event, which is listed and can be downloaded here below.

List of necessary documents:

In case of death
  • Death certificate in copy (certified copy or stamped and signed by the Branch Manager for compliance with the original for individual policies and copy stamped and signed by the Branch Manager for group policies).
  • Coroner’s certificate of the fact of death (which can also be issued by a hospital doctor or the family doctor, according to the case), in copy.
  • Certificate of inheritance or succession certificate (for policies where heirs are not specified) or Disposal of guardianship if heirs are under aged.
  • Document from the Police regarding the causes and circumstances of death (in case of death due to accident)
  • Copy of the medical record from the family doctor or in case it cannot be obtained due to objective grounds, a medical certificate issued by the family doctor.
    Each page of the medical record will be numbered, signed and stamped by the family doctor and will bear the mention „true copy”.
  • Any other documents that GARANTA ASIGURARI may consider necessary to evaluate the insured event.
In case of maturity
  • Maturity indemnity payment request signed by the Contractor, as previously provided by GARANTA ASIGURARI.
  • Copies of the beneficiaries’ ID.
  • Copy of the insurance policy.
  • Any other documents necessary to settle the claim file.
In case of disability
  • Copy of the insured person’s medical record from the family doctor or in case it cannot be obtained due to objective grounds, a medical certificate issued by the family doctor*.
    Each page of the medical record will be numbered, signed and stamped by the family doctor and will bear the mention „true copy”.
  • Copy of the discharge summaries.
  • Copy of all medical leaves.
  • Any other documents that GARANTA ASIGURARI considers necessary to evaluate the insured event.
  • In case of disability, GARANTA ASIGURARI will request the proof of disability or on-going disability by means of documents issued by competent authorities (Medical certificates proving the disability state, Medical resolution regarding the ability to work, Retirement decision due to disability, Certificate of disability level, Statement of the attending doctor to certify disability or any other document that GARANTA ASIGURARI considers necessary in order to establish the right to compensation).
In case of critical illnes
  • Copy of the insured person’s medical record from the family doctor or in case it cannot be obtained due to objective grounds, a medical certificate issued by the family doctor.
    Each page of the medical record will be numbered, signed and stamped by the family doctor and will bear the mention „true copy”.
  • Any other documents to prove the medical history.
  • Copy of the discharge summaries.
  • Any other documents that GARANTA ASIGURARI may consider necessary to evaluate the insured event.
In case of waiver of premium due to disability
  • Copy of the discharge summary/ies and/or medical letter/s which clearly indicate the hospitalization period.
  • Copy of the resolution regarding the ability to work or of the disability level certificate, where the case may be, issued by the social security expert doctor or, if the case, by the medical-military survey committees connected to the hospitals in the national defense, public order and national security system.
  • Copy of the discharge summary/ies in case the Insured was hospitalized due to an event which lead to Total Permanent Disability.
  • Documents issued by the competent authorities, which prove the accident circumstances, where the case may be.
  • Any other documents requested by GARANTA ASIGURARI and necessary to settle the claim file.
In case of daily hospital allowance
  • Copy of the discharge summary/ies and/or medical letter/s which clearly indicate the hospitalization period.
  • Any other documents that GARANTA ASIGURARI considers necessary to evaluate the insured event.

The insurance indemnity is collected within 30 days since the date the claim file has been completed with the last document requested.
